01 / The care model
From a routine consultation to ongoing family support, the proposed services are designed to work together when clinically appropriate.
The planned palliative-care unit will support symptom relief, nursing care, psychosocial wellbeing and family education, including appropriate end-of-life care.
15 planned palliative beds
Within a proposed 40-bed facility. Capacity remains subject to premises assessment and approvals.
Outpatient consultations, chronic-condition reviews, preventive assessments and coordinated referrals.
Geriatric assessments, supervised adult day care, mobility support and respite for caregivers.
Physiotherapy, functional rehabilitation, laboratory testing and imaging through licensed services or partners.
Clinically suitable home nursing visits, caregiver training, employer programmes and sponsored patient care.
